Excellent question. With good basic setup, it is hard to get infected. I tuned my setup for over a decade, helping the dumbest of friends ; )

Realtime:
Avast Home (current version 7)

On-Demand:
(Run Cleanup! by Gould)
Remove existing antivirus (it's obviously failing!) (see: http://scforum.info/index.php/topic,7956.0.html)
Combofix first, then
Concurrent scan of Hitman Pro & Superantispyware, then
Cleanup with Hitman Pro, then
Cleanup with Superantispyware, then
<some bootable cd / dvd with virus scan? Let me know ;)>, then
Install Avast Home, update, let it quick scan, schedule complete offline scan and let it run overnight...


Browser:
Chrome - S A N D B O X ! ! ! ! NO INTERNET EXPLORER. NO FIREFOX. Maybe Opera. Maybe PaleMoon x64 (Firefox)

OS:
All Windows-based OS's

Additions...
Some custom scripts to cleanup temporary files whenever I log a user on. Slows boot, but very effective in virus spreading. ASK if you'd like to know how and what...
NO OUTLOOK or other local email clients. Web-based email (in Chrome) ONLY.
If given away for free (happens once in a while...), Zemana Antilogger. Never bothered me and hardly ever interrupts - still testing insists it's one of the best in its genre...
